Liability Release
All participants who wish to engage in activities on the premises must read, agree to, and complete this document prior to the commencement of any activity.
By checking the box below, I agree that Powerslam Ltd., doing business as Los Banditos ("Los Banditos"), is in no way responsible for the safekeeping of my personal belongings while I attend class.
I understand that classes may be physically strenuous, and I voluntarily participate in them with full knowledge that there is a risk of personal injury, property loss, or death. I agree that neither I, my heirs, assigns, nor legal representatives will sue or make any other claims of any kind whatsoever against Los Banditos or its members for any personal injury, property damage/loss, or wrongful death, whether caused by negligence or otherwise.
I understand that I am participating in a sport that involves physical contact. I assume full responsibility for all of my actions during and connected to my training at the Los Banditos Gym. I recognize the risks inherent in participating in this type of sport. Therefore, I hereby release, waive, and discharge Powerslam Ltd., DBA Los Banditos, and all persons associated with Powerslam Ltd., DBA Los Banditos, including but not limited to its owners, officers, agents, employees, and volunteers, from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, while participating in physical activity or while on or upon the premises where the activity or contact is being conducted.
I, the undersigned, also state that to the best of my knowledge, I am in good health and physical condition and know of no reason why I cannot participate in this training. I further represent that I have no medical or other condition that would expose myself or others at Los Banditos to any type of risk while participating in classes, instruction, or use of the facilities at Los Banditos.
Emergency Authorization and Media Release
I further certify I am at least 18 years of age. If under 18, my parent/guardian must sign below. In case of emergency, I hereby authorize any licensed medical personnel to perform any accepted medical procedure deemed necessary, and I agree to bear any potential expenses of such treatment. I also agree that my attendance and/or performance at training may be photographed, filmed, or taped and used for marketing purposes by Los Banditos, and I waive any compensation thereof.
By checking this box, I acknowledge that I have read the release above and agree to all of its terms. I understand that checking this box constitutes a legal signature confirming that I acknowledge and agree to the terms of this agreement.
