If you’re in charge of a group of skilled people or have been part of a large workforce, you may have had regular staff evaluations. Most companies with long-term staff members conduct periodic employee performance reviews. Employee performance reviews are an essential part of management and human resources. They also contribute to the success of a business, as well. Regular reviews of employees or staff members foster an ethical work environment. It also makes sure that employees’ achievements and growth are recognized. You can use this template to make sure that your team and the company are moving in the right direction with its most important asset—its employees.

This template will help you conduct and manage employee performance reviews. Maintaining records of employee performance assessments can get really messy, really quickly. This is especially true at a company with thousands, or even hundreds of thousands, of employees. If you’re a manager or human resources professional, you can begin by populating Departments, Titles, Reviewers, and the Employees table with contact information for team members. Organizations often have annual, biannual, or even quarterly reviews—all of which can be performed using this template. The frequency can be set in the Review periods table. Once all employees and review periods are set, you can utilize a form called Create new employee review, in the Reviews table, to facilitate entering all of the necessary staff assessment data. The Reviews table contains predefined assessment criteria, which you can tailor to your needs. Let’s jump into the template’s highlights and more details, by table:

Employees

This table contains all the staff members’ basic contact information. All employees or staff members that are to be reviewed or appraised should be included in this table. The Employees table is linked to all the other tables in this database. Records under this table are referenced by the ID number field. Each record in this table can be linked to multiple records in the Reviews table, so each staff member can have more than one assessment.

Fields

  • ID number. An autogenerated primary key that serves as the unique identifier for this employee.
  • Last name. The employee’s last name.
  • First name. The employee’s first (given) name.
  • Middle initial. The employee’s middle initial.
  • Title. This field is linked to the Titles table, indicating the employee’s current title or position in the organization.
  • Group. This field looks up the employee’s group based on the employee’s title. It enables grouping of employees according to their subteams.
  • Department. This field is linked to the Departments table, which indicates the department the employee belongs.
  • Review periods completed. This field looks up all the employee’s reviews based on the employee’s data in the Reviews field. It lists all of the reviews the employee had.
  • Phone number. The employee’s current phone number.
  • Email. The employee’s email address.
  • Number of reviews. This field counts all the employee’s reviews.
  • Remarks. This field is for recording any notes or remarks about the employee.
  • Is active. A checkbox indicating whether or not the employee is still active or under the reviewer’s supervision.
  • Last modified. A date and time stamp that indicates the employee’s record’s last modification or update.
  • Reviews. This field links to the Reviews table, which matches up the employee to respective reviews.
  • Full name. A formula field that shows the employee’s full name.

Views

  • All employees. Displays all employees without any filters sorted by Full name in alphabetical order from A to Z.
  • Add new employee. This is a form view for the purpose of simplifying the entry of basic contact information for employees.
  • Active employees. This is a gallery of all active employees stored in the Employees table.
  • Employees with assessments. Displays all employees or staff members with reviews sorted by Full name in alphabetical order from A to Z.
  • Employees without assessments. Displays all employees or staff members without reviews sorted by Full name in alphabetical order from A to Z.

Departments

Unless you’re part of a very small company or startup, you probably have departments at your organization. Departments fall into categories such as finance, legal, human resources, marketing, sales, and more. These disciplines are standard at midsize and larger companies, each working together within and across other departments to carry out business functions.

Fields

  • Department name. The name of the department.
  • Active reviewers. A filter formula that displays only the active reviewers from the Assigned reviewers link field.
  • Assigned reviewers. A linked field for assigning department-specific reviewers.
  • Is active. A checkbox that indicates whether the department is active or not.
  • Employees. A linked field gained from the Employees table indicating all employees assigned to the current department.

Views

  • All departments. Displays all departments sorted in alphabetical order from A to Z.
  • Active departments. This is a gallery of active departments arranged in alphabetical order from A to Z.

Titles

If you’re in a company, then you have some sort of role and title. This table allows for the grouping of employees according to their position, level, or title within the organization. You can customize this table to group employees by subteams. You can have teams of employees with almost similar responsibilities. As an example, provided in the template is a predefined set of titles for a software development team.

Fields

  • Title. The title, level, or position name.
  • Group. A selection field indicating which group or subteam the title belongs to.
  • Description. A description of the title, which may include position specifics and responsibilities.
  • Members. This field looks up for a list of employees using this title based on this table’s linked field, Employees.
  • Number of members. A formula field that counts the number of employees using this title.
  • Employees. This field is linked to the Employees table, identifying all employees or staff members using this title.

Views

  • All company titles. Displays all the titles or staff designations in an organization without any filters sorted by Group in alphabetical order from A to Z.
  • Business analyst titles. This is a gallery view of titles filtered by Group that is Business analysts.
  • Quality analyst titles. This is a gallery view of titles filtered by Group that is Quality analysts.
  • Software engineer titles. This is a gallery view of titles filtered by Group that is Software engineers.

Review periods

The Review periods table serves as a lookup and lists the applicable review periods required by an organization. Feel free to modify this table and incorporate your organization’s practices. This table grows as the organization spans more periods or years.

Fields

  • Review period. A field indicating a review period that has been text-formatted according to the organization’s practices or standards.
  • Reviews. A field that allows the Reviews table to reference this look-up table.
  • Number of employees reviewed. A formula field that displays the number of staff members reviewed in this review period.

Views

  • All review periods. Displays all review periods.
  • Review periods. This is a gallery view displaying all review periods.

Reviewers

Populate this small contacts table with reviewers, managers, and supervisors tasked to conduct employee assessments.

Fields

  • Full name. This is a field for the reviewer’s full name.
  • Phone number. The reviewer’s current phone number.
  • Email. The reviewer’s current email address.
  • Department. This field is linked to the Departments table identifying departments the reviewer is currently assigned.
  • Is active. A checkbox field that indicates whether the reviewer is still active or not.
  • Reviews. This field is linked to the Reviews table indicating all reviews conducted by this reviewer.

Views

  • All reviewers. Displays all reviewers without any filters sorted by Full name in alphabetical order from A to Z.
  • Active reviewers. This is a gallery of all active reviewers arranged in alphabetical order from A to Z.

Reviews

The Reviews table is the template’s heart. It intends to include all reviews for each employee in the Employees table. An employee can have many assessments depending on their organization’s practice. A form view called Create new employee review is available to make the process of creating a review more user-friendly. This table contains predefined employee evaluation criteria as individual fields or columns. Bear in mind that if you choose to add or remove criteria, you must also update the Rating total (max 50) and Rating average fields’ formulas.

Fields

  • Review ID. An autogenerated primary key that serves as the unique identifier for this review.
  • Employee ID number. A selected employee ID number identifying the employee reviewed in this record. You may refer to the Employees table and its Active employees gallery for reference.
  • Employee name. This field looks up for the employee’s full name based on the selected employee ID from the Employee ID number field.
  • Review period. This field is linked to the Review periods table indicating the review period where this review belongs.
  • Assessment score. A formula field stating this review’s overall review rating similar to the Huntington Ingalls Rating Scale.
  • Rating average. A formula field that gets the average of this review’s ratings under all review criteria.
  • Rating total (max 50). A formula field that gets the sum total of this review’s ratings from all review criteria.
  • Attendance and timekeeping. This is a sample assessment criterion with the description found in the table’s form view.
  • Creativity and innovation. This is a sample assessment criterion with the description found in the table’s form view.
  • Problem solving and resourcefulness. This is a sample assessment criterion with the description found in the table’s form view.
  • Communication skills. This is a sample assessment criterion with the description found in the table’s form view.
  • Interpersonal skills. This is a sample assessment criterion with the description found in the table’s form view.
  • Customer experience. This is a sample assessment criterion with the description found in the table’s form view.
  • Work ethic. This is a sample assessment criterion with the description found in the table’s form view.
  • Productivity. This is a sample assessment criterion with the description found in the table’s form view.

  • Summary and recommendations. A summary of this assessment and, at the same time, allows the reviewer to include employee recommendations based on this employee’s review.
  • Supporting documents. This field allows managers or reviewers to attach supporting files related to the employee’s review.
  • Reviewer. The assigned manager, supervisor, or reviewer who created this record or made the employee review.
  • Last modified. A date and time stamp that indicates the employee’s review’s last modification or update.

Views

  • Create new employee review. This is a form view for entering employee evaluations or reviews performed by the reviewer.
  • Reviews 2020/Q1. Displays all reviews for the sample review period, 2020/Q1. You can continue to add a view per review period.
  • Reviews 2020/Q2. Displays all reviews for the sample review period, 2020/Q2. You can continue to add a view per review period.
